Use this form if the checkout payment couldn't verify you — for example, the card is in someone else's name, or your plan didn't involve a card charge. Print it, fill it in by hand, sign it, and send a photo or scan to [email protected] from the email address on your Family Rumpus account, or mail it to 2 Bridgeview Cir, Ste 8 #267, Tyngsborough, MA 01879. We'll confirm by email once it's recorded, and only then can children join your family.
